It’s time to start preparing for the annual Chemical Inventory Submission! 

Does this apply to my lab and what is it?  The responsible party and/or designee for each WVU laboratory and chemical storage area is required to submit annually a current, accurate chemical inventory.  The chemical inventory must be generated electronically using the instructions found with the form above.  The chemical inventory form has changed, please make sure that you use the updated excel spreadsheet that is located on our website. Do not modify the sheet only one lab per form. If a new 704 door posting needs to be generated please fill in H7 on the new form. It is also important to provide after hour phone numbers (Cell/Home Phone) in case of emergencies.   

When is it due and where does it go?  The chemical inventory is due to your CHO by July 1st of each year.  Your CHO will forward your inventory to WVU Environmental Health and Safety at by July 15th.  However, if you are a 9-month appointment, then your chemical inventory is due in May prior to your departure.

Why is this required?  State, federal and local regulations require that this information be collected annually.  Specific quantities of certain chemicals trigger reporting requirements to various agencies.  The chemical inventory is also used by local emergency response teams in the event of an emergency response to your lab.
