Supervisors are to report injury and incidents within 24 hours for needed review and advisement from Environmental Health and Safety and Medical Management. Please clearly complete, sign and send by one of the following methods:

Student - Visitor Accident Report Form

To be completed by the student or visitor to report an injury. Form must be completed within 24 hours. Form must be printed, signed, and emailed, faxed or mailed to:
