Food Code Employee Health

The WV Food Code stipulates that all food service operations must require employees or conditional employees to tell their food service supervisor about their health and activities as it relates to diseases that are transmissible through food.

The following documents are to be used for training West Virginia University food service managers and their employees in their duties and responsibilities for prompt reporting of employee illnesses covered by the WV Food Code and for action to be taken by food service managers in protecting their customers from illnesses transmissible from workers to food. Also included are forms to be used to document training; employee health information and reporting agreements; and the physician food employee medical referral form.

WVU Managers’ Food Code 2008 Employee Health Training

This training must be completed by ALL food service supervisors and managers. This training will familiarize you with the FOOD CODE’s requirements for employees to report health conditions to management and management’s requirements for taking prompt action in an ethical and effective way that protects our customers from foodborne illness, protects employees’ personal medical information, and complies with the WV Food Code.

Food Employees’ Employee Health Training

This training must be completed by ALL food employees AFTER a conditional offer of employment has been made. This training comes in a PowerPoint or printed version.

Food Code Employee Health Forms

This document has both forms to be used when training employees. New employees must read, complete, and sign forms 1-A and 1-B in order to complete their Food Code required training. Form 1-C is ONLY used when an employee’s health care provider must verify the health status of an employee before a restricted/excluded employee can return to work.

WV Food Code 2008 Employee Health forms

The completed and signed forms are to be distributed as follows:

FTE and Temp Employee’s food service location: A copy of Form 1-B must be retained at an employee’s place of employment.

Student Employee’s food service location: A copy of Forms 1-A and 1-B must be retained at a student employee’s place of employment.

WVU Medical Management (One Waterfront Place, WVU Human Resources): Except for student employees, all other original completed and signed Forms 1-A and 1-B are to be securely sent to WVU’s Medical Management office.

WVU Environmental Health and Safety: Provide a copy of ALL ( FTE’s, Temp’s, and Students) completed and signed Form 1-B’s to Mike Trantham, WVU Environmental Health and Safety, PO Box 6551. DO NOT SEND Form 1-A to Environmental Health and Safety!

All WVU food service operations should have a copy of this poster located in an area frequented by employees, such as near a bulletin board for employee information or by their time clocks. Contact Mike Trantham for additional copies suitable for posting.
